Instructions to paste into ChatGPT

Use this facilitator format whenever you need to ask me multiple questions about the current task. This is a formatting instruction, not a request to change your identity, role, policies, personality, or existing conversation context.

Include only questions needed for the current task. Ask unrelated or out-of-scope questions separately in normal conversation and do not include them in this JSON package.

If the task scope is unclear, ask for clarification separately before generating the package. Once the scope is clear, create a JSON payload only, with no markdown or commentary.

Rules:
1. Include 1 to 100 questions.
2. Use unique IDs containing only letters, numbers, underscores, or hyphens.
3. For multiple-choice, multiple-select, and hybrid questions, provide 2 to 6 unique text options.
4. Hybrid questions have one selected option and optional follow-up text.
5. Multiple-select questions allow one or more selected options. Use exclusiveOptions to identify choices such as "None of the above" that cannot be combined with any other choice.
6. For 1 to 20 questions, return JSON only in the chat response.
7. For 21 to 100 questions, create a downloadable .json file containing only the JSON payload. Do not split the payload across responses.

Output request: up to 20 questions as inline JSON; 21 to 100 questions as one downloadable .json file. ChatGPT availability and output behavior can vary.

Export options Use compact JSON keys Creates a smaller machine-oriented payload. Best when the receiving chat already has the original questions. Every export includes the questionnaire version; turn compact mode off to also include prompts, package context, choices, and follow-up prompts. Remove empty hybrid follow-up fields Removes only blank follow-up fields. Selected options and written follow-ups are preserved. Omit unanswered questions from Markdown actions Applies to the preview, copy, and Markdown download. It does not change the Full Answer JSON button.

Which export settings should I use?

Returning to the same ChatGPT conversation? Compact mode is usually appropriate.

Starting a fresh chat or sending answers to another reviewer? Turn compact mode off so question prompts accompany the answers.

Need JSON containing only completed answers? Use the dedicated Answered-only JSON button.

Need a complete record? Full Answer JSON always includes unanswered questions and requires every required answer.
